Dissertation Title Submission Form for Doctoral Students

Instruction

Doctoral students need to complete this form when completing paperwork for the degree application. The deadline for submitting a dissertation title is listed on the Graduate School Calendar.

The information supplied on this form is used by the Registrar's Office to include the doctoral student in the commencement ceremony program. The student must be listed in the program to be part of the ceremony.
